Position Summary:

The PRN RN Intake Assessor plays a critical role in evaluating new clients to determine eligibility for non-medical home care services. This role involves performing assessments, developing initial care plans, and ensuring that all client needs are clearly communicated to the care team. As a PRN position, scheduling is flexible and based on agency needs.

Key Responsibilities:

• Conduct comprehensive in-home assessments for prospective clients to evaluate physical, emotional, and environmental needs.
• Determine appropriateness for non-medical home care services and identify any referral needs for other services.
• Create an individualized care plan that reflects the client’s personal needs and preferences.
• Educate clients and families on Victory Home Care Agency’s services, care procedures, and expectations.
• Complete all intake documentation accurately and timely, in accordance with agency protocols and applicable regulations.
• Communicate findings to the care coordination and administrative team to ensure smooth client onboarding.
• Provide nursing oversight of care plans and update plans as clients need change, if required by agency policies or state regulations.
• Comply with all HIPAA guidelines and maintain client confidentiality.
• Provide intake coverage on a PRN (as-needed) basis, including evenings and weekends if needed.

Qualifications:

• Current and active RN license in the state of OH.
• Minimum of 1 year of nursing experience, preferably in home care, geriatrics, case management, or intake assessment.
• Knowledge of non-medical home care services and client eligibility requirements.
• Strong assessment, communication, and documentation skills.
• Ability to work independently and travel to client homes as needed.
• Reliable transportation, valid driver’s license, and proof of insurance.
• CPR certification.

• Flexible availability and reliability for PRN assignments.

Pay: $30.00 – $40.00 per hour
